FAQs

Assurity is a trusted carrier providing coverage that's a source of stability when you need it most. For the past 130 years, they have specialized in a variety of supplemental health insurance products. Assurity will handle your parents’ claim submissions and provide them customer support from a live representative to answer any questions they may have.

FACTS will provide a marketing kit that you can use to communicate the key benefits of the coverage and how parents can apply.

No, FACTS will not have access to information about which parents purchase coverage or which children coverage has been purchased for. Assurity will maintain that information.

UNICO is an insurance broker located in Lincoln, Nebraska. Due to state regulations, a writing agent is required. UNICO acts as that agent to allow the policy to be issued.

Parents can apply for coverage online. It only takes a few minutes.

Benefits are based on the treatment received by the insured for a covered accident. Assurity provides a wide range of coverage for common medical treatments. Some benefits may provide coverage up to $25,000.

Absolutely. Since this is supplemental coverage, how much or how little a parent’s health insurance coverage has paid for a medical treatment does not affect the coverage provided by Assurity. Even if a family has reached their deductible and out-of-pocket maximums for the year, Assurity will still pay a benefit for a covered accident.

Unfortunately, some accidents can lead to the death of the insured. The policy contains an accidental death rider which pays a death benefit resulting directly from an injury sustained in a covered accident within 180 days of the accident. This coverage may also assist parents in covering additional costs associated with funeral arrangements.

There are many options on the market when it comes to student accident insurance. Some of the key differences with the Assurity product are:

  1. A modern online application and claim submission process that is quick and easy.
  2. Parents can choose monthly or yearly premium payment. Premiums can be made via credit card or ACH deposit.
  3. There are no exclusions for specific high-contact student sports (i.e. football).
  4. Parents and students can continue to carry the policy beyond school.
  5. The policy does not have to be renewed each school year.

This is an individual policy in which the parent purchases and owns the policy directly from Assurity on behalf of their child(ren). Neither the school nor FACTS are involved in the agreement between the two parties.

Assurity will provide all customer support to the parent regarding coverage or claim questions. Assurity representatives can be reached via phone at 800-869-0355 or by email at [email protected].

Kids of all ages are active, and accidents happen. With today’s deductibles and out-of-pocket expenses with health insurance, accident insurance can assist in offsetting the costs that a parent is responsible for. This also provides an additional level of coverage to assist parents if they experience unexpected medical costs due to a covered accident while still having to make their tuition payments.

The parent has the flexibility to choose how long they would like to have coverage. The policy will continue to renew until they decide to cancel.

Yes, a parent can cover as many children as they wish. However, since this is an individual policy, a parent must submit a separate application for each child.

Yes, the parent may continue to pay for the coverage for as long as they wish.

Yes, a parent may purchase coverage for each child. Since this is an individual policy, the parent will need to purchase a separate policy for each child.

It only takes a few minutes, and a parent can submit claims online at MyAssurity.com.

A parent will need to provide a copy of a document(s) that supports the claim, such as an itemized bill from a medical provider. Some standard examples of documents accepted include an HCFA 1500 form or a UB-04 form, both available from a medical provider.

If a claim is approved, the parent can choose to be paid either by check or by direct deposit to their checking or savings account.

The parent may contact Assurity at 1-800-869-0355 to discuss the cancelation of the policy.
